The market is expanding as patent expiries create opportunities across high-value biologics. Keytruda and Keytruda Qlex generated about USD 31.7 billion for Merck in 2025, and Merck says biosimilar competition could begin in December 2028, when the primary compound patent expires. More than ten developers already have pembrolizumab biosimilars in clinical trials, and Amgen has submitted an FDA application for a nivolumab biosimilar. Ustekinumab, aflibercept, and denosumab shaped the 2024–2025 wave, while omalizumab, pembrolizumab, and high-dose aflibercept are driving the next development cycle. The FDA had approved a total of 90 biosimilars through the end of 2025 and several checkpoint inhibitor programs are moving toward potential commercialization.
Pricing and pharmacy rules decide who wins after approval. The FDA approved 18 biosimilars in 2025, and interchangeable products can be substituted at the pharmacy counter where state law allows.
